Please give the full names you wish each member of the household or family to be identified by in the book, using caps or lowercase as you prefer — and if these are not legal names, please give those, too, for reference. While we hope that real names can be used for the most part, requests for confidentiality will be honored by the use of last initials or a pseudonym or nickname.

If there are non-slave members (boy, pet, lover, spouse, and so on), please list them, too (indicating the role or relationship), though the focus of the profile will be on the Master and (male) slave(s).
